  • Form ADV Part II + Schedule F

    Form ADV is a two-part document that advisers use both to register as investment advisers with the SEC and to disclose certain details of their practice to current and prospective clients. Part I is the document they submit directly to the SEC; Part II includes specific information on the adviser's services, fees and investment strategies.

  • Confidential Data Gathering Questionnaire
    The Confidential Data Gathering Questionnaire is to be completed by you prior to the "Get Acquainted Meeting". So that we can have meaningful discussion at this first meeting, please fax your completed Questionnaire to 860-606-8030 prior to the meeting date. We also ask that you bring supporting documents (such as account statements) to the meeting so that we can begin to analyze your situation and develop a prioritized plan of objectives.
